Definition

Intervention is a specified strategy to individualize learning for a student or a group of students. It addresses three needs: remedial (students not working at grade level), practice (students on grade level who need additional practice) and enrichment (students who already displayed mastery). Intervention occurs on 3 levels. The classroom level addresses needs for specific objectives. The building and community levels address needs for entire subject areas.

Goal

The student will demonstrate 70% mastery of the cumulative Academic Content Standards (ACS) in math and language arts.

Intervention Plan Review

The effectiveness of the intervention plan will be assessed at the conclusion of each quarter. The intervention team will determine the effectiveness of the intervention plan using the following criteria: grade card, anecdotal records, classroom teacher and tutor observations, building principal and parents.
Based on the Intervention Team's assessment an appropriate course of action will be set for the next quarter.
Performance Level Course of Action
Student maintains a “C” average Continue with established intervention plan
Student exceeds a “C” average Revise the intervention plan to more closely correspond with normal classroom requirements.
Student falls below a “C” average Assess the specific areas of difficulty and increase the modifications appropriately.

Brief description of student's classroom and standardized test performance.

Intervention Strategy

This section defines the role of the classroom teacher, tutor, and parent(s). Classroom, assignment and testing accomodations are also identified.

Objectives

This section contains specific objectives the student will attain.

Parental/Guardian Consent

We have read and agree to the above intervention plan. I understand the intervention plan will be reflected on my child’s report card.
