4210 Cumulative Health File
4211 Required Immunizations
4211.1
XCS requires a record of the dates of immunizations for measles, rubella, hepatitis, TB, polio, diphtheria, and tetanus before students will be allowed to attend school.
4211.2
The immunization record must be signed by a physician.
4211.3
Immunizations must be current or appointments scheduled to administer needed immunizations for student to continue attendance at XCS.
4211.4
Any student from a country designated as high-risk for TB must take a new TB test (mantoux skin) prior to enrollment.
4211.5
Immunization records are recorded in the student's cumulative health file.
4212 Health Screening Tests
4212.1
XCS organizes hearing, speech, and vision screening tests for all kindergarten students and other referred students.
4212.2
Results from any screening tests are shared with parents and recorded in the student's cumulative health file.
4213 Release of Health File
4213.1
XCS will release information to requesting agencies or schools only after the parent signs a form authorizing such release.
4213.2
All student accounts must be paid in full before XCS will release any records.
