Consultation Check List:
To the Consultant: Go over this sheet with the applicant and explain your position.
- ___This advisor recommends that this ministry team application move forward in
the process to Department Director Conference(s).
- ___The advisor has specified his or her recommendations for revision below.
- ___The advisor has made a copy of the application worksheet with suggested
revisions, and a copy of the summary sheet for his or her own records.
- ___The advisor will forward a letter summarizing the recommendations to the next advisor.
- ___The advisor will be available for Consultation #3.
- ___If the applicant wishes to go forward, I have collected the $50.00 application
fee.
For the Department Director Conference, the applicant should call to make an
appointment at 823-6500:

- ___The applicant understands the purpose of the next step in the process.
- ___The applicant understands that the revisions should be made and the notebook
given to the next advisor at least three days before the scheduled
appointment.
- ___The applicant understands that he or she should also call the next advisor
the day before the appointment to confirm.
Due to the considerations specified below, it is this advisor's recommendation
that we defer this ministry accrediting application at this time.
- The advisor has made a copy of the application worksheet with suggested
revisions, and a copy of the summary sheet for his or her own records.
- The advisor has also made a copy of the summary sheet and given it to the applicant
for their records and consideration.
- The advisor will keep this notebook and return it to the Ministry NetWorking
Department.
- If the applicant understands the recommendations and the reasons for them. I also
understand that I have two options to decide from at this point:
- I can choose to agree with the recommendation for deferment, discontinue the accrediting
process of my own choosing, and try again at a later date, or
- I am not certain that I agree with these recommendations at this time and would like
further consideration.
- ___I (the applicant) will call the Ministry NetWorking Department and inform them of my
decision. They will direct me from that point.
