I. Description
1. In your own words, describe your ministry.
- Explain what your ministry is currently doing, the types and number of meetings.
2. Please give us a Mission Statement for your proposed ministry team.
- Very briefly, state the purpose of your proposed team; Try to state the vision you have
for this ministry and its goals in one or two sentences.
3. Briefly describe how this ministry developed (include significant contributors from
the past, and whether they are involved today or not).
- If your ministry is a new ministry, how did you develop the burden for this ministry?
Include names and dates of key people involved in starting the ministry.
Credentials
1. What training and experience do you have that qualify you to be the team leader of
this ministry?
- Include the highest level of education you have completed, including classes taken
through Xenos, workshops and conferences. Also include the length of time in each
experience. [Ministry leaders must have the expertise to do what they propose. This
requirement could vary for different kinds of ministries.]
2. What else can you tell us about your past in ministry?
- Include all of your past ministry experiences in Xenos and outside of Xenos.
3. Do you have a secular job? If so, please describe it, including your hours, pay,
length of your tenure there, and duties.
- Self-explanatory [Leaders are often prevented from attaining success because of
conflicts with the demands of their work. Also, a bad work record could point to future
problems in ministry]
4. Are you willing to quit your secular job if you are needed in the ministry and the
church is willing to support you?
- Self-explanatory [If a ministry team grows, as some have, to include hundreds of
participants, it will likely need staffing. We want to know if the senior leader will be
available. Our pay levels are known to be low compared to secular jobs.]
What percentage of your income did you give to God's work last year?______ To Xenos
General Fund?________ Are you a member of the Xenos Fiscal Support Team?
Give the percent of your gross (before taxes) household income given to each category. [At
Xenos, we developed a problem where a large proportion of our leaders were not giving to
our own church. We have decided this needs to change, at least to the extent specific in
the rules for joining the Xenos Fiscal Support Team]
6. Estimate your total indebtedness from all sources other than a house.
- Please itemize by category [We have lost quite a few leaders because of financial
pressure, often caused by debt. Also, excessive debt could point to personal problems that
need attention.]
7. Would you have any objection to taking a polygraph test if the need arose?
- This might be required for your protection if we receive any accusations, especially in
working with children. [We have never asked anyone to take a polygraph, but we could do
so, and in any event, we would be curious about anyone who said they are not willing.]
8. Have you ever been convicted of a crime? If yes, please explain what & when.
9. Have you ever been laid off or fired from a job? If yes, please explain.
- Self-explanatory [Character issues are often involved in firings]
10. Have you ever stolen anything from your place of employment? If yes, please
explain.
11. Are you married? If so, what is your spouses' name, daytime phone number, and what
relationship will he/she have to this ministry?
- Self-explanatory [We have had problems with divided interests in ministry. If a
couple will not join in the same ministry, we wonder how demanding the ministry will be.
Low demand ministries will be less likely to cause problems here.]
12. How does your spouse feel about this ministry?
- Self-explanatory [Spousal opposition to the ministry is a deadly negative]
13. Do you have any children? How Many? Ages?

14. Do you have scheduled "family time(s)" with your spouse and/or
family? If so, how frequently?
[We will do a fairly extensive study of the stability of the family life of the
senior leader and other leaders, because of the requirement that leaders be good managers
of their home life. This study is based on these questions and the personal references
given elsewhere.]
